This is a brilliant idea. Not only they need proper guidance in terms of preparing for the exams, but also someone to foot the hefty bills. If the news is spread amongst various alumni in US, I hope this idea will only grow stronger.

My understanding and experience is that to get an
externship/observership you have to be physically present
in US and keep calling them. If you a friend OR relative they may help you. I also believe that its very difficult to obtain externship while being in India.

Having said that I would recommend you to still try....prepare a CV and letter and send to various hospitals and see what respond you get. Good Luck!

I wanted to add my 2 cents while Sanjeev Boss has rightly advised Siddiqui. While getting an externship is difficult, it is not impossible. But once you get it, your chances in matching with a good program in the field of your choice is manifold higher.

While I don't know the magic mantra for getting the externship, you need to prepare a great CV (project yourself, but don't lie), define your interests in a particular field of medicine to do your residency in, put that in a cogent manner and start shooting off the mails to various programs all over the US asking for their policy regarding externship. For every 99 program that does not respond to you, you might get an answer. Follow the lead and keep corresponding with the best of your abilities.

Of course externships are a hard-toil, no-pay thing. But if you can get one, there is nothing like it.

Remember communication is the key!

Another route, slightly longer but allows more option is to apply for Masters in Public health (MPH), work for 1-2 yrs , also get attached to different reaserch programs and work yourself up.
Pros:
- you can work and get paid for your tution, daily living etc.
- You have the oppertunity to meet all the big guys in a university program and get attached for either clinical or non clinicl research. This eventually MIGHT lead you into a residency.

Cons:
- Longer time.

I met two guys from manipal, who are in this process, one of them landed a reidency at Baylor neurology ( an unheard feat)

Key word is: Nothing is impossible here. You can come with with very lousy scores and still get into a good program. It is importatnt to be here and SELL YOURSELF.

so i want to try to get clerkships( medical school rotations of 4-8 weeks); clerkships r available only to those whoa are in final year of medical school......i think they r easier to get than externships...... what do u say???
is it worth applying to them???
like mayo, mssm-NY, they offer clerkships in many branches....like in mayo they mention on site that they have 400 seats and around 700 ppl apply. it does depict less competition than externships........but most programs need STEP 1 scores....

Honestly I dont know about clerkships here but even if they want to take you what I dont know if they will sponsor your visa for it.

As I said If you are here things are easier so whatever way you can........ come here first.

As Hemant said about MPH, other area is research...there is no negative side to trying. In today's e-mail age you can apply anywhere and whatever you want.

One thing we have no control and cannot help is 'visa'

i searched and searched on internet but was lost.from the net i founs that i will have to buy malpractice insurance for different states ( is this true?) and they charge case to case basis for malpractice insurance fee!

I didn't know RKP went moochless! He would be looking funny for sure :) When I saw him May last year, he still had his whiskers on.

It appears that you all are going to have a gala organizing this alumni meet. It is indeed commendable. I was surprised to know that there is a PMCH alumni association of NA. Would it be a good idea to compile the alumni database and have it up on a website (a PMCH website I mean). It has been a long time but our PMCH website went defunct after opening its shop on the net within a few months. So another effort that we should direct ourselves towards is getting a website up and going. Sid seems to be a web enthusiast and only he has been conferring with us all on this site. Can he take up the initiative and get us a web address. I would suggest that we should pool in some money for that purpose.

If any of us is going to Patna around CFD time, I would suggest an informal meeting with the students out there to counsel them about USMLE and other career options (I though there is a whole bunch of them still preparing for PG entrance exams for last several years). I would encourage people who have recently gone to MMST program in IIT-KGP to also join us.

As a matter of fact,I would be Patna on a 10 day leave in 2nd week of February and would be happy to meet anyone who would want any sort of guidance on USMLE.

Hope to hear from some of you at least.

Many have wanted to build a directory of PMCH alumni. I personally know atleast 30 + friends and senoirs who are settled here. We have enough space on our server to create a large data base program. Our future plans include creating a tab for Alumni directory and a second tab to incorporate all PMCH links available on search engines.

Abhay, it is a good initiative on your end to meet with the students and encourage them of USMLE. Also thanks for talking to Rohan.

We have met two new MAMC graduates and their family. The interesting side to the story is : They are doing Radiology fellowship @ MD Anderson without having to go through a formal radiology resideny in US. If they are able to manage 4 yrs of fellowship training without a break, they will be board eligible to practice radiology. There is so much useful infirmation floating.

Sid, I like your idea of putting this concept on CFD magazine. How is the medicine dept? Any changes since VPS became the HOD. Is he computer savvy. Do you know if he has an email. Does PMCH have a projector and a laptop for powerpoint presentation setup for class room teaching ? By the way we are still waiting for your photographs. Film dhulne mein kafi waqt lag raha hai.... I in fact know of quite a few people who are going through the fellowship only route to get their accreditation with ABNR. I think there is more scope for Interventional Radiology for IMGs with good scores (like 90+) and some research to show for (IR is less sexy than compared to other fellowships in radiology). People have managed to get into Neuroradiology as well.

I guess people in PMCH have also gotten quite net savvy (chat savvy, surf savvy if you may say it). Laptops are commonplace, so is broadband. Digging the right information mine remains an issue and I am sure people like Sid are doing that as well.

I would love to meet the people in PMCH in an informal session if they require any information. Information is POWER and we all need to understand that.

It would be a good idea to get in touch with RKP through local junta out there who could coax him into getting a PMCH website (I was surfing and saw a nearly decent website of Bhagalpur Medical COllege!). VPS being the HOD medicine would understand the idea. and possibly help implement it too.

I had recently been to a Bihar meet in Jacksonville, Florida where I met a few PMCH alumni (from 70s and early 80s) and then it seems there has been a huge gap before between those batches and even now people come in trickles. Getting a directory to show for on this website or the PMCH official website would be helpful in letting the current crop know that you can do whatever you want (including USMLE and get a residency)- in other words, Impossible is Nothing.

previous site pmch.net was built mainly by dr prashant jha. i was in 1st year and approached him for my services. later he was accused of bungling funds(i think it was a false accusation) by his co-workers infront of dr vijay prakash sir. which led to the site's downfall.

i contacted him thru email 2 years ago. he was in canada then. i asked him that i want to carry on his work on site as the site has become defunct. he met me when he came to patna , we had a hat fore 3 hours. he said that if u want to get involved better be careful as money bungling accusations. he told me he had returned all assets( site's ID & PW , a desktop computer etc. to dr vijay prakash sir.

i being an final year medicine examinee, am too scared to meet dr vijay prakash sir regarding website. so i was thinking that i firt develop the amateur website and then approach dr vps sir.

sorry if i discussed a good old campus story in a blog.

dr vps is very motivated for college and academics. medicine dept now has a fully functional endoscopy unit, the new platinum jubilee audi has audio visual laptop lcd system. every dept has its own laptop and lcd and teaches who make ppt slides can use them intheir lectures... very few teachers though use them......hum log ko achcha hai, nahi to notes utarte utarte slide hi next ho jaat hai *smile*

pmch is still not netsavvy. those who are still float around in the world of orkut/ chatting......guys dont know how to dig for info.
i have been motivating most ppl for usmle, even nmch friends so that we can form a study group and do the bestest.

nice site of bhagalpur, but not developed on frontpage, maybe dreamweaver..... http://jlnmcbgp.org but its professionally developed and pictures of students dont really appear as the real bhagalpurian studs......hmmmmm...........just my tuppence.

actually itna exam chalta hai ki time hi mil paata hai....

does doing summer research projects in medical sciences etc carry any advantage in resideny applxn. ICMR sponsors 2 month researcx in home institution... i have done one in dept of path relating to prevalence of head neck cancers in pmch.....planning to do one moe this year.
what i wanted to know is that Are they really worth wasting 2 months on???????
on other blog i heard these 2 months summer researh projects are very very minute things and dont really add to your CV. research in usa is the research the residency ppl consider.! is this true
what if i try to publish them, but i dont think i can do that in indexed journals......

It is so good to see you bubbling with ideas. I know Prashant Jha very well and I would go by your impression of him. He was in Patna when I had last been to India in May and I had spoken with him as well. You could take a cue from him in getting the website up and running. It is a great idea to host a PMCH website. There are several domains which could do so. If we have few netizens in PMCH who could maintain the website the site would work without much professional help. PMCH alumni association should help out with funds if the college administration is apathetic to the needs of getting this done. It is high time they do so besides only helping organize the alumni meet during CFD. You could also mention your experience in building the college website in your CV!

I am wondering if all the laptops/av system provided by the college is through one particular company/organization and if they could help in getting the website done as well.

But you must focus on your final year exam before you go any further. Also every bit counts. By doing the ICMR projects you are doing something very commendable, something that I did not know of at all during that stage of my career. By doing the Head Neck cancer prevalence thing, you could build a database and then start looking retrospectively for local factors causing their incidence. If you are planning on continuing the same project, it would be something to look into. If not go ahead and still do the other project. When you are writing your CV, you can genuinely mention your research experience as this would be funded by ICMR which is more known than many other organizations here in US.

In fact, more students from PMCH should do this during their summers - besides eating mangoes and going to watch movies first day late night shows!

I would be happy to speak with you and other people- in an informal session in the college canteen or in the RSB auditorium (Canteen ki chai abhi bhi utni hi ghatiya hai;). I guess our alumni association should take initiative in getting some USMLE related practice materials for the students out there so that they can read from it in the library itself without having to buy them. We used to have so much of a tough time getting the whole thing.

I would let you know dates of my stay in Patna soon.

If the pmch.net domain still belongs to college, one could easily get it restored and functional if VPS allows it. Such web site should probably be managed by professional companies purely to prevent what Prashant had to go through. Additionally these are dedicated souls who are expensive. I am also sure the alumni or PMCH campus can afford to sustain this cost.

We had organised LKP '95 and were able to literally rob the streets of ashok rajpath for funds. From the money saved we purchased full audio system, including mikes, speakers and amplifiers etc, so that we did not have to rent it again. Last I heard it was used for commercial purposes. All of us are willing to help as long as IT WORKS.

When we purchased our domain, we bought enough space, so that down the road we could buy subscription for USMLE qs and load it on our domian. Many kaplan books and audio MP3 are available for FREE aka PHREE from different P2P clients.

Sid, your ICMR research project is a good idea. As you have learnt we include 2 Neurologist, 2 Internist, 1 radioloist, 1 Endocrinologist, 1 Infectious disease and 1 Critical care specialist. Some of these are faculty in Univ programs. If you find a case, interesting images, patient pictures, one of us can easily help you get published either as case of week, NeuroImages or communications. Find the cases. It is the intent to publish that counts.

2 years ago, in CFD 2006, with the help of organising batch-2001, we initiated the CINE SOCIETY with our own funds; in it we used to show 2 movies every evening during the whole CFD week in the audi using DVD player and LCD. and it became the most popular society after LKP. even the common room society and its evening badminton, carrom matches were deprived of their crowds)
.......the company couldnt even maintain their supplied laptops....

* yeah final exams do matter.....im still scared...does marks matter a lot????????

3)abhay boss! give me atleast 2 weeks notice b4 your visit, so that i can spread the word and if possible ask for the use of auditorium..........that would be best...............
during CFD audi would be busy and the environment is anti-studies & full masti; so b4 16th feb would be better.....

canteen ki chai to abhi bhi 'jhel' hai....;-) magar majboori hai, peena parta hai.

4)regarding materials, now in india they r no problem........as AIIMS bookstore makes sure they r supplied pirated at low cost
but even though one set of rare USMLE books, for reference in library in an almirah could be a good idea. i think it is possible to donate an almirah with books, as recently Dr D Singh(OBGYN) had donated alimrah full of OBGYN journals. but it is to be thought of later.

5)the pmch.net domain is active upto 2008/9, i think as prashant boss told me! and it cost a lot to buy it.

6) i have seen the old audio system of audi.......but didnt know its source!.......gr8 of u guys.
now the audi has new audio system, cordles mic, collar mic.
in CINE society, we used the output from DVD player and put it into the amplifier using an self improvised device. the result was excellent......though not dolby stereo with the six installed 'AHUJA' speakers in audi, but our satisfaction was more than what dolby could have given......

there is a recent joke from medicine unit.
3 new interns joined under unit chief....he asked each of them 'tum kahan se ho? and commented on each of them!
one from muzaffarpur. chief replied. "arrey wahan, wahan to khali khidki mein sheesha lagta hai, parhai hota nahin"
second from south. "tumko kya aata hoga!, wahan to clinical seekh hi nahi paata hai sab, language prob"
third from pmch "tumse to poochna bekar! tumhara to janmsidh adhikar hai kuch nahin aane ka!"

I will not even comment on the chief who said such words about PMCH docs. Insipte of the unexisting infrastructure....strikes...."pairvi's/pouwa's"....castism....without any help from the administration, the laurels that PMCH grads achieve is commendable. Imaagine the heights they will achieve if they are given an environment similar to AIIMS/MAMC.

Having said that I would only ask from each PMCH graduates once they pass out - "Dont ask what PMCH can do for you but ask what you can do for PMCH" ...... dont give me any accolades for this as I have stolen it from the current political fever in the US....actually it was said by JFK.

I am sorry, I was out of the loop for few months.
It appears that there will be a big shortage of Internal Medicine/Primary care physician in next few years as a big chunk of Internal Medicine Doctors are opting to be Hospitalists. Please spread the word that the things in USA is going to be much better for some one willing to take the risk and come here.
However, it also appears that the candidates are getting much higher scores in USMLE these days! So study hard, study right and come over. Humlog hain na.
